Default passenger side door is off the track

Guys I have a 1998 Honda Civic EX my problem is my passenger side Windows is screwed up. The window goes up and down without any problem but the window is off track so my question is what causes this and how do I get it back on track. Basically what parts and tools do I need in order to straighten things back out.

Start by lubricating the rubber side channels with silicone lubricant and adjusting the window glass as described in the service manual (see sites 1 and 2 in my signature).
