Default Hesitation for the first block

When I start my car and drive off it hestatates for a block or two, meaning it will lurch some and if I punch it all the way down I get barely any power.

It won't do this if I sit and rev it a bit before driving off.

Its done this for basically forever, and by that I mean tens of thousands of miles.

Head gasket recently replaced, no fix there.

There is no smoke when it does this.

It has had several tune ups since the problem started.

I think tis about time I fixed this bs.

1997 Civic HB
Manual Trans.
1.6 non-vtech
237,000 Miles

ps. My brother's 1994 Civic LX 5 speed also does this, but worse.
