94 si Engine knock

94 si Engine knock
Friend has a 94 civic SI, 394,000km has a knock in the engine, but only in drive, doesn't happen in N or P.
We are in the first stage of trying to fix this problem and was wondering if anyone might happen to have a few tips.
Going to get computer codes today to see if that helps. There is a random CEL but we think it's from a vacuum leak in one of the lines.
Any help is better then no help.
Thanks

auto trans, canada,
code 43 - fuel system supply... makes sense for another problem, but not the knock.
head is currently off, can't start the car right now. we found a grinding/rubbing coming from what sounds like the top of the timing belt or cam
any other ideas?

Check the motor mounts especially those at the front and back.
Spin the water pump and timing belt tensioner by hand, should be almost silent and no wobble.
Otherwise you'll have to put it back together and run it to try and localize the noise.
